Application

Eligibility criteria for the Home Repair Program:

• 
  
  Applicant must own the home where the repairs are to be made.
  
  
• 
  
  Applicant must occupy the home as their primary residence.
  
  
• 
  
  Applicant must live in home for 6 months before applying for assistance.
  
  
• 
  
  Applicant or a member of their household must demonstrate a need that makes the requested
  repairs necessary.
  
  
• 
  
  Applicant’s household income must fall below 80% of the Area Median Income.
• 
  
  Applicant receiving home repair services must make their home accessible during repair work. Any
  able-bodied household member will be asked to assist in completing the project when applicable.
• 
  
  Applicant must demonstrate willingness and ability to contribute a down payment on a sliding scale based on their income.
  

Income Verification Checklist: 

• 
  
  Return three months’ worth of income verification for all adults living in the household for the most recent three months. 
  
  • •  
    • Including check stubs, child support verification, social security statements, etc.
    • Note: Please request a statement of benefits for social security and disability income from the social security administration office at 800-772-1213
      
• 
  
  Return six months’ worth of bank statements.
  
  
• 
  Return your most recent tax return.
• 
  Other documents may be requested as needed.
